A complication of valve replacement by a caged-lens prosthesis.

چکیده
SINCE the introduction of artificial heart valves, investigators have been continuously modifying them, endeavoring to eliminate complications associated with their use. -3 The Starr-Edwards ball-valve prosthesis was the first to be widely accepted; however, problems due to its weight, cage size, and resistance to blood flow have stimulated the search for a better prosthetic device. In spite of this effort, the ideal heart valve has not been developed. Recently, a "caged-lens" design has become the prosthesis of choice in some centers. One type, Cross-Jones, consists of a titanium frame, a silicone rubber lens-shaped disc, and a woven Teflon fixation flange. The freely floating disc is held in position over the valve seat by a cage of three titanium legs3 (fig. 1 left). The silicone rubber lens is reinforced by a titanium ring molded within it. The ring prevents excessive deformation of the lens, and, being 0.010 inch larger in diameter than the valve orifice, it prohibits the lens from slipping through the orifice under high closing pressure.

متن کاملLate outcome of mitral valve replacement with the Cross-Jones prosthesis 36 years after initial surgery.
A 60 year-old woman with rheumatic mitral stenosis underwent re-replacement of Cross- Jones caged lens mitral valve prosthesis, 36 years after valve implantation. In 1968, she underwent mitral commissurotomy. In 1992, she had a stroke, and in July 2009 echocardiography revealed the malfunction of the prosthesis with pannus and reduced mitral prosthetic area < 1.0 cm(2) with the elevated transpr...

Introduction: Prosthetic valve thrombosis is a rare and severe complication of valve replacement, most often encountered with a mechanical prosthesis. The significant morbidity and mortality associated with this condition warrant rapid diagnostic evaluation. Although surgery is the first-line therapy in symptomatic obstructive mechanical valve thrombosis, thrombolytic therapy has been used as a...

he ideal prosthetic valve would have excellent hemodyamics (similar to a normal human valve in the same osition), last a lifetime, be free of structural dysfunction or reakdown, and require no particular medical therapy such s anticoagulation. Needless to say, such a valve is yet to be vailable. However, valvular replacement is still relatively oung—the first successful aortic valve replacement...

Prosthetic valve re-operation has greater mortality and morbidity than primary valve replacement. By recognition of factors influencing on causes of redo operation and preoperative survival, one can select appropriate prosthesis at primary valve replacement and when operation performed at appropriate time, surgical risk can be reduced. Methods and Materials: Two hundred patients that underwent p...
